Hi, all,

I attached a v2 which should address the feedback I got during the
call yesterday and in this e-mail thread. Page 1 of the attached PDF
has text descriptions and page 2 has the entity-relationship diagram.

Please let me know if there is anything I missed or if you have any
further comments.
